There are four types of video lottery licenses.

Establishments, which must have an on-sale alcohol beverage license, are the businesses where the terminals are located.

Operators own the terminals and provide maintenance using technicians certified by the Lottery.

Distributors provide the terminals and parts to the operators.

Manufacturers produce the terminals after the hardware and software have been tested and approved by the Lottery and an independent gaming laboratory.


The following checklists will help you complete the application process. The forms must be printed, completed, signed, and, in some cases, notarized.

Once you have completed the forms, please send all of the forms and your application fee to the South Dakota Lottery, 711 E. Wells Avenue, Pierre, SD 57501. Call the Lottery at (605) 773-5770 if you have any questions or want more information about becoming a video lottery licensee.

Establishment License Checklist

Applicants for an Establishment License must:

  1. Complete the Application for Video Lottery form Application for Video Lottery Form PDF, Application for Video Lottery Instructions PDF
  2. Complete Individual History Forms for each individual listed in Section 8 of the Application for Video Lottery form Individual History Form PDF, Individual History Form Instructions PDF
  3. Complete Investigation Authorization Forms for each individual listed in Section 8 of the Application for Video Lottery form Investigation Authorization Form PDF, Investigation Authorization Form Instructions PDF
  4. An application fee of $50 is required and must accompany your application documents. Please make checks payable to the South Dakota Lottery

Note: Law requires that a licensed video lottery establishment be a bar or lounge.

Operator License Checklist

  1. The same information as an Establishment listed above
  2. Provide Additional Operator Information PDF
  3. Complete the Authorization for Electronic Funds Transfer (EFT) Form EFT Form PDF, EFT Form Instructions PDF
  4. Submit Uniform Surety Bond or Irrevocable Letter of Credit or cash deposit or certificate of deposit with the Lottery endorsed as a payee Uniform Surety Bond PDF, Irrevocable Letter of Credit PDF

Distributor License

Applicants for a Distributor License must submit:

  1. The same information as an Establishment listed above; and
  2. Provide Additional Distributor Information PDF

Manufacturer License

Applicants for a Manufacturers License must submit:

  1. The same information as an Establishment listed above; and
  2. Application for Manufacturer Form PDF
  3. Certification PDF
  4. Manufacturer Requirements PDF

Note: Pursuant to SDCL 42-7A-50, information provided in license applications is considered confidential information on file with the South Dakota Lottery except:

-Business name, business address, and business phone number; and -Name of any video lottery operator including, if the video lottery operator is a partnership, the name of any partner and, if the video lottery operator is an association or corporation, the name of any director, any officer, and any stockholder who owns five percent or more of the stock in the association or a parent or subsidiary corporation.
